What is PCNL Surgery?

PCNL (Percutaneous Nephrolithotomy) is a minimally invasive keyhole surgery to remove large kidney stones that are too big for laser treatment.

1
Anaesthesia & Positioning
Patient placed face-down under general anaesthesia. A ureteric catheter is placed via the bladder to fill the kidney with contrast dye for precise X-ray guidance.
2
Percutaneous Access — 1cm Entry
Under fluoroscopy (live X-ray), a needle is passed through the skin of the back (flank) directly into the kidney. A guide wire is inserted, then the tract is dilated to 1cm. A nephroscope (camera) is passed through this keyhole tract into the kidney.
3
Stone Fragmentation
The nephroscope directly visualises the kidney stone. A pneumatic or laser lithotripter breaks the stone into small fragments. Very large stones are broken systematically and removed in sections.
4
Stone Removal + Nephrostomy
Stone fragments are flushed out or removed with forceps. A nephrostomy tube (small drain) is placed temporarily in the kidney to allow healing and drainage. X-ray confirms no residual stones.
5
Recovery — 2–3 Days Hospital
Nephrostomy tube removed on day 2. Ureteric stent removed 2–4 weeks later (minor OPD procedure). Back to light work in 1 week, full activity in 2–3 weeks. Post-op KUB X-ray to confirm stone-free status.

Which Surgery for Your Stone Size?

The right procedure depends on stone size, location, and composition. Our urologist will advise after reviewing your CT scan and reports.

< 5mm
Very Small
Watch & Wait
High chance of passing on its own with fluids. Medicine to relax ureter (tamsulosin). Monitoring by urologist.
5–20mm
Small–Medium
URSL Laser
Ureteroscopic laser lithotripsy — no incision, same-day discharge. Best for ureteric and small kidney stones.
20–40mm
Large Stone
PCNL / Mini-PCNL
Gold standard for large stones. 95%+ stone-free rate in one sitting. Covered under Ayushman Bharat.
40mm+
Staghorn / Complex
Multi-tract PCNL
Staghorn stones filling the kidney — PCNL with 2 tracts. Expert surgeon essential. Available at Satyadev Hospital.
Important: Stone size alone doesn't decide the procedure. Location (ureter vs kidney pelvis vs calyx), stone composition (hard calcium oxalate vs soft struvite), kidney anatomy, and overall kidney function all influence the choice. Always get a CT KUB and urologist consultation before deciding. Call 091222 37338.

Types of PCNL Surgery

There are three sizes of PCNL — the urologist selects based on stone size, kidney anatomy, and patient factors.

Standard PCNL
Sheath: 24–30 Fr · Best for 25–40mm+
The original PCNL — uses a 24–30 Fr nephroscope. Most efficient for clearing very large stones and staghorn calculi in a single sitting. Highest stone-free rate.

Mini-PCNL
Sheath: 14–20 Fr · Best for 15–25mm
Smaller nephroscope through a smaller tract — less bleeding, shorter hospital stay, faster recovery. Best for moderately large stones where URSL is not ideal but standard PCNL is too aggressive.

Ultra-Mini PCNL
Sheath: 11–13 Fr · Best for 10–20mm
Very thin nephroscope — almost no bleeding, often tubeless (no nephrostomy drain needed). Bridge between URSL and Mini-PCNL. Excellent for lower pole stones difficult to reach with URSL.

Our approach: We do not push one procedure over another. Dr. Ranjan reviews your CT KUB, stone analysis report, and kidney function — and recommends the procedure that gives you the best stone-free outcome with the least risk. Patients with 20mm+ stones who chose URSL elsewhere and had residual stones are seen regularly here for PCNL completion. Call 091222 37338.

Complete Kidney Stone Treatment at Satyadev Hospital

We offer every kidney stone procedure — so your treatment is decided by what's best for you, not what the hospital happens to have.

1
Standard PCNL
Percutaneous Nephrolithotomy for stones 20mm+. 24–30 Fr nephroscope, highest stone-free rate. Best for large stones, staghorn calculi, and lower-pole stones inaccessible by laser.
✅ Ayushman FREE · 95%+ Stone-Free
2
Mini-PCNL
Smaller 14–20 Fr sheath — less blood loss, faster recovery, shorter hospital stay. Best for stones 15–25mm where standard PCNL is not required. Same stone-free efficiency with fewer side effects.
✅ Ayushman FREE · 2–3 Days Hospital
3
URSL Laser (Ureteroscopic Lithotripsy)
Laser fibre passed through the urethra — no incision, same-day discharge. Best for ureteric stones and kidney stones up to 20mm. Holmium laser breaks stones into fine dust for easy passage.
✅ Ayushman FREE · Same-Day Discharge
4
RIRS (Retrograde Intrarenal Surgery)
Flexible ureteroscope passed into the kidney — reaches stones in difficult locations (upper pole, lower pole calyces). Laser fragmentation in situ. No incision. Alternative to PCNL for some patients.
Flexible Ureteroscopy
5
CT KUB + Stone Analysis Workup
Non-contrast CT KUB is the gold standard imaging for kidney stones — shows exact size, location, density (Hounsfield units), and relation to adjacent structures. Essential before PCNL planning. Done in our diagnostic centre.
Pre-Surgical Planning
6
Ureteric Stent Insertion / Removal
JJ stent (double-J stent) placed after PCNL or URSL to ensure ureter heals without blockage. Removed 2–4 weeks post-surgery in a quick OPD cystoscopy procedure. No anaesthesia required for removal in most cases.
Post-Op Care
7
Stone Prevention & Metabolic Workup
24-hour urine collection, serum calcium, uric acid, parathyroid hormone — to find why stones are forming. Diet and fluid advice, stone-preventing medications, and long-term monitoring to prevent recurrence.

"I had a 28mm kidney stone which three hospitals said needed open surgery. Dr. Ranjan did PCNL — a small hole in my back, 60 minutes, 2 days in hospital. Zero stone residual on X-ray. I cannot believe how smooth it was. Ayushman Bharat covered everything — not a single rupee from my pocket."

Frequently Asked Questions — PCNL Surgery Patna

Common questions from patients considering PCNL surgery at Satyadev Hospital Patna.

What is PCNL surgery and when is it needed?+
PCNL (Percutaneous Nephrolithotomy) is a minimally invasive keyhole surgery for large kidney stones. A 1cm incision is made in the back, a nephroscope (camera) is passed directly into the kidney, and stones are broken and removed. PCNL is recommended for: stones 20mm or larger, staghorn calculi (stones filling the kidney), lower pole stones difficult to treat with laser, stones that have failed other procedures, and hard stones (calcium oxalate monohydrate) resistant to laser. Performed by Dr. Kumar Rajesh Ranjan M.Ch. Urology at Satyadev Hospital Patna. Call 091222 37338.
What is the cost of PCNL surgery in Patna?+
PCNL surgery normally costs ₹55,000–₹1,00,000 in Patna depending on stone complexity and hospital stay. At Satyadev Hospital Patna, PCNL is fully covered under Ayushman Bharat PMJAY — eligible patients pay nothing for surgery, hospital stay, anaesthesia, or post-op medicines. CGHS and ESIC are also accepted. For cash patients, pricing is transparent with no hidden charges. Call 091222 37338 to discuss costs and insurance before visiting.
What is the difference between PCNL and Mini-PCNL?+
Standard PCNL uses a 24–30 Fr nephroscope and is best for very large stones 25mm+ and staghorn calculi. Mini-PCNL uses a smaller 14–20 Fr instrument — less blood loss, shorter hospital stay (2–3 days vs 3–4 days), faster recovery. Both achieve similar stone-free rates. The choice depends on stone size, density, and kidney anatomy. Dr. Ranjan performs both at Satyadev Hospital Patna. Both are covered under Ayushman Bharat. Call 091222 37338 for a personalised recommendation.
Is PCNL safe? What are the risks?+
PCNL has a very good safety profile when performed by an experienced M.Ch. Urologist. Risks include: bleeding (1–2% require blood transfusion), infection (managed with antibiotics), adjacent organ injury (rare with imaging guidance), residual stone fragments (second-look procedure if needed). The risk is significantly lower with an experienced surgeon. Dr. Ranjan has performed 5,000+ urological surgeries. The risk of NOT treating a large stone — obstruction, infection, kidney damage — far outweighs surgical risk. Call 091222 37338.
How long does recovery take after PCNL surgery?+
After PCNL at Satyadev Hospital Patna: Day 1-2 in hospital with nephrostomy tube — removed day 2 or 3. Discharged day 3 or 4. Light work in 1 week. Ureteric stent (JJ stent) removed 2–4 weeks post-surgery in a quick OPD cystoscopy (5 minutes, usually no anaesthesia). Full physical activity in 2–3 weeks. Post-op KUB X-ray at 4 weeks confirms stone-free. Drink 2.5–3 litres of water daily. Follow-up CT at 3 months if clinically indicated. Call 091222 37338 for detailed post-op guidance.
What reports should I bring for a PCNL consultation?+
Please bring: (1) CT KUB scan (non-contrast) — essential for PCNL planning; plain X-ray KUB and ultrasound are supplementary but not sufficient alone. (2) Blood reports — CBC, BMP (creatinine, urea, electrolytes), liver function. (3) Urine culture report if available. (4) Previous kidney stone reports, procedure notes, or discharge summaries. (5) Current medication list. (6) Ayushman card and Aadhaar if using PMJAY. Urology OPD: New patients 11AM–1:30PM, Mon–Sat. Call 091222 37338.
