What Is an AV Fistula?
An arteriovenous (AV) fistula is a surgical connection between an artery and a vein, usually in the forearm. This creates a larger, stronger blood vessel that can handle the high blood flow dialysis requires — typically 200–400 ml per minute. It is considered the gold-standard vascular access for haemodialysis — lower infection risk, longer-lasting, and better blood flow than the alternatives.
Comparing the Three Types of Dialysis Access
Why Timing Matters So Much
A fistula takes 6–8 weeks to mature — sometimes longer, and occasionally a first attempt doesn't develop well and a second site needs to be tried. If your nephrologist expects you'll need dialysis within the next 3–6 months, getting the fistula created now avoids having to rely on a temporary catheter later. Catheters carry a meaningfully higher infection risk and are meant only for emergency or short-term use — starting dialysis through a catheter because a fistula wasn't planned in time is one of the most preventable complications in kidney care.
The Full Procedure — Step by Step
Pre-Surgery Vessel Mapping
An ultrasound (Doppler study) checks that your artery and vein are suitable in size and blood flow for a fistula.
Day-Care Surgery
Usually done under local anaesthesia, connecting the artery and vein in the forearm or upper arm. Takes about 1–2 hours.
Initial Healing (1–2 Weeks)
The surgical site heals; mild swelling and soreness are normal. Avoid heavy lifting with that arm.
Maturation Period (6–8 Weeks)
The vein gradually enlarges and thickens ("matures") to withstand repeated needle insertions. Gentle exercises (like squeezing a stress ball) can help this process.
Ready for Use
Once matured and confirmed by your surgical team, the fistula is used for every dialysis session going forward.

Caring For Your Fistula After Surgery
- Check daily for the "thrill" — a buzzing vibration confirming blood flow
- Keep the site clean and dry, especially in the first two weeks
- Do gentle exercises (stress ball squeezing) once cleared by your surgeon
- Report redness, warmth, swelling, or pain immediately
- Allow blood pressure checks or blood draws on the fistula arm
- Sleep on or compress the fistula arm
- Carry heavy bags or lift heavy weights with that arm
- Wear tight jewellery or clothing on the fistula arm
